Thank you for your interest in exhibiting at our 36th
annual seminar, a 2 day symposium aimed at providing grass roots
perspective on current state-of-the-art practice and emerging concepts
in pulmonary and sleep medicine. Attendees at this event include
respiratory therapists, nurses and other health care professionals from
throughout the tri-state western PA area.
Please be aware the conference, exhibits and other festivities have
moved to a new location this year and will be
held at the Four Points by Sheraton Pittsburgh - North hotel.

Once you have reviewed the prospectus information, payment can be made
by check (see prospectus for instructions) or by credit card using the

CREDIT CARD PAYMENT
Booth Rental ($575) -
All inclusive vendor
cost includes draped booth, table, 2 chairs & electricity. Rate
also includes registration and all breaks served in Exhibit area and
Thursday evening festivity for a maximum of four (4) company representatives per
booth.

Two Booth Rental ($925)
- All inclusive vendor cost includes two draped booths, two tables, 4
chairs & electricity. Rate also includes registration for
continuing education hours from event, all
breaks served in Exhibit area and the Thursday festivity for a maximum of
six (6) total company representatives.

Additional Company Representative
($50 each) -
Additional company representatives may be added.
Rate includes registration for all
breaks served in Exhibit area and the Thursday festivity.
However, if the additional representatives are seeking continuing
education credit, they are required to register for the conference using
the normal attendee member or non-member registration rate by
